The stair rail system was not equipped with a mid rail resulting in an opening of approximately 38 inches between the stair and the bottom of the hand rail.  b) Stairs to 1st floor upper level: On or about September 2, 2021 and time prior, an employee used the stairs to access the first floor upper level to install structural steel.  The stair rail system was not equipped with a mid rail resulting in an opening of approximately 36 inches between the stair and the bottom of the hand rail.  c) Stairs to 2nd floor upper level: On or about September 2, 2021 and time prior, an employee used the stairs to access the 2nd  floor upper level to install structural steel.  The stair rail system was not equipped with a mid rail resulting in an opening of approximately 36 inches between the stair and the bottom of the hand rail.
