29 CFR  1926.416(a)(1): Employees were permitted to work in close proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:  Site - On or about September 3, 2021, an employee was exposed to electrical shock hazards while working in close proximity to the electrical drop lines while engaged in roofing activities where the electrical lines were not deenergized or insulated to prevent the hazard.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  Jobsite - On or about September 3, 2021, where an extension ladder was used as access to and from the roof above the patio area, the side rails did not extend at least 3 feet above the upper landing surface, exposing employees to a fall of more than 10 feet.

29 CFR  1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  Jobsite - On or about September 3, 2021, each employee installing shingles on a 5 pitch roof was not protected from falling over 16 feet to the ground where 2 employees were observed not utilizing fall protection.  Pento & Sons Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1224221, citation number 1, item number 1b and was affirmed as a final order on June 26, 2017, with respect to a workplace located at 7-9 Amherst St. in Nashua, NH.
