29 CFR  1926.28(a): The employer did not require the wearing of appropriate personal protective equipment in all operations where there is an exposure to hazardous conditions.  MCAS BEQ Project, Building 5697, 1st floor: Employee(s) conducted chilled water system pipe insulation operations and used Armaflex 520 Adhesive a hazardous Chemical. Employee(s) wore white cloth and Maxiflex gloves which were not sufficient to reduce exposure to the chemicals hazardous properties.  Employees were exposed to chemical exposure through the skin.

MCAS BEQ Project, Building 5697, 1st floor: Employee(s) conducted chilled water systems pipe insulation operations and had used Armaflex 520 Adhesive, a hazardous liquid chemical. Employee(s) wore prescription and/or standard safety glasses  which were not sufficient to reduce exposure to the chemicals' hazardous properties.  Employees were exposed to eye and face chemical exposure.

29 CFR  1910.1200(h)(3)(iii): Employee training did not include the measures employees can take to protect themselves from chemical hazards, including specific procedures the employer had implemented to protect employees from exposure to hazardous chemicals, such as appropriate work practices, emergency procedures and personal protective equipment to be used: (Construction Reference: 1926.59)  MCAS BEQ Project, Building 5698: Employee(s) used hazardous chemicals such as Armaflex 520 Adhesive  to complete insulation installation work. The employer had not provided training specific to the health and safety hazards of that product at the time of initial assignment specific to chilled water systems insulation work. Employees were exposed to chemical hazard(s).
