29 CFR 1910.95(b)(1):When employees were subjected to sound exceeding those listed in Table G-16, feasible administrative or engineering controls were not utilized: On or about September 23, 2021 employees were exposed to noise hazards in excess of the permissible exposure limit (PEL). a). The employee operating the corrugator (wet end) was exposed to continuous noise levels at 259% of the permissible daily exposure (8-hour, time-weighted average sound level of 90 dbA or equivalently, a dose of 100 percent), during the 466 minute sampling period. Exposure calculations include a zero increment for the 14 minutes not sampled. b). The employee operating the corrugator (wet end) was exposed to continuous noise levels at 277% of the permissible daily exposure (8-hour, time-weighted average sound level of 90 dbA or equivalently, a dose of 100 percent), during the 468 minute sampling period. Exposure calculations include a zero increment for the 12 minutes not sampled. c). The employee operating the corrugator (wet end) was exposed to continuous noise levels at 194.4% of the permissible daily exposure (8-hour, time-weighted average sound level of 90 dbA or equivalently, a dose of 100 percent), during the 470 minute sampling period. Exposure calculations include a zero increment for the 10 minutes not sampled.

On or about September 23, 2021 employees were exposed to noise hazards in excess of the permissible exposure limit (PEL). a). The employee operating the corrugator (wet end) was exposed to continuous noise levels at 259% of the permissible daily exposure (8-hour, time-weighted average sound level of 90 dbA or equivalently, a dose of 100 percent), during the 466 minute sampling period. The employee operating the baler was exposed to continuous noise levels at 82.0% of the permissible daily exposure (8-hour, time-weighted average sound level of 85 dbA or equivalently, a dose of 50 percent), during the 459 minute sampling period. Exposure calculations include a zero increment for the 21 minutes not sampled. This employees last audiogram was June 26, 2019.

29 CFR 1904.10(a):Where an employee's hearing test (audiogram) revealed that the employee had experienced a work-related Standard Threshold Shift (STS) in hearing in one or both ears, and the employee's total hearing level was 25 decibels (dB) or more above audiometric zero (averaged at 2000, 3000, and 4000 Hz) in the same ear(s) as the STS, the employer did not record the case on the OSHA 300 Log: On or about September 23, 2021, the employer failed to record a work-related standard threshold shift (STS) on their OSHA 300 logs. Around or about 6/26/2019, the employer was provided information that an employee had a work-related STS, but the employer did not include the STS on the OSHA 300 logs for the year 2019.
