29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Lab Cut Up Area: On or about September 10, 2021 and at times prior, employee were exposed to amputation hazards while unjamming MDF trimmings from operational Table Saw A. The employer failed to ensure employees performing this operation were utilizing the company lockout tag out procedure.

29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: a) Lab Cut Up Area: On or about September 10, 2021, and at times prior, employee were exposed to amputation hazards while unjamming MDF trimmings from operational Table Saw A. The employer failed to ensure that employees were utilizing machine specific shut down procedures prior to performing clearing and maintenance on the equipment.
