29 CFR  1926.501(b)(13):Each employees engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  (a) AH Construction LLC at 1508 East 69th Street North, Sioux Falls, SD 57104:  On or about and at times prior to September 15, 2021, the exposing employer did not ensure that employees were protected from falls while conducting roofing activities on a residential home.  Employees were conducting re-roofing operations on a 6-in-12 pitched roof with no fall protection in place but no employees were utilizing the systems. This condition exposed employees to fall hazards of approximately 9 to 21 feet.  AH Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1440305, citation number 1, item number 1, and was affirmed as a final order on 12/27/2019, with respect to a workplace located at 5th and Main Street, Lennox, SD 57039.  AH Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1354712, citation number 1, item number 1a and was affirmed as a final order on 11/7/2018, with respect to a workplace located at 1200 South Hyannis Port Lane, Sioux Falls, SD 57106.
