29 CFR 1910.215(a)(2): Abrasive wheel(s) used on grinding machinery were not provided with safety guard(s) which covered the spindle end, nut, flange projections:  a) Work Area/Bay 5 - On or about September 13, 2021, the employer did not ensure that abrasive wheel machinery, such as the Challenger by Wilton bench grinder, Model 1534, was equipped with safety guards to cover the spindle end, nut, and flange projections.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.215(a)(4): Grinding machinery was not used with work rest(s) to support offhand grinding work and/or was not adjusted closely to the wheel with a maximum opening of one eighth inch:  a) Work Area/Bay 5 - On or about September 13, 2021, the employer did not ensure that a work rest was installed to support grinding work on abrasive wheel equipment, such as the Challenger by Wilton bench grinder, Model 1534.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  On September 15, 2021, the employer did not ensure that all electrical equipment was installed and used in accordance with the listing and labeling:  a)	Break Room - A blue extension cord was connected in series (daisy chained) to supply power to a power strip. The power strip was used to supply power to a mini-refrigerator and a microwave.  b)	Work Area/Bay 6 - An orange extension cord was connected in series (daisy chained) to supply power to a 30 foot Electricord Retract-O-Matic retractable extension cord reel.   c)	Work Area/Bays 2, 4, and 6 - Electrical junction boxes, which are intended to be mounted to a wall and wired with permanent conduit, were not mounted to a wall or fixed location and were used as electrical drop downs to power portable hand tools and the AC machine.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
