29 CFR  1910.132(a):Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:  a. Ohio Polytech, LLC - Bucyrus, Ohio: On or about September 21, 2021, the employer did not ensure employees were protected from burn injuries caused by a buildup of gas and/or plastic that is released when the slide plate is operated on Extruder #1.  Employees were exposed to burns from ejected plastic heated to approximately 300-360 degrees in that personal protective equipment for the face, head, and upper body/extremities was not provided and used for screen changes.    b. Ohio Polytech, LLC - Bucyrus, Ohio: On or about August 24, 2021, the employer did not ensure employees were protected from burn injuries caused by a buildup of gas and/or plastic that is released when the slide plate is operated on Extruder #2.
