Beal Avenue, Bucyrus, OH: On or about February 13, 2022, the employer did not ensure employees were protected while performing unjamming activities at the Titan Conveyor of the "Shredder" Line. The employer did not ensure that the energy control application steps to control the electrical, hydraulic and gravitational sources were implemented prior to unjamming product materials from the conveyor belts.
