29 CFR  1910.178(a)(4):Modifications and additions which affect capacity and safe operation shall not be performed by the customer or user without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly.  On or about September 23, 2021, at an establishment located at 1100 East Green Bay Street in Shawano, Wisconsin, in the Service Center; employees utilized a forklift with a fork attachment that was modified and not approved by the forklift manufacturer prior to use.  The left fork was modified by the employer who had drilled a hole in it in order to attach a trailer hitch ball to allow employees to move boats and trailers into the Rig Room for servicing.

Except as provided in paragraphs (f)(7) and (f)(8) of this section, the employer shall ensure that each container of hazardous chemicals in the workplace is labeled, tagged or marked with product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals, and which, in conjunction with the other information immediately available to employees under the hazard communication program, will provide employees with the specific information regarding the physical and health hazards of the hazardous chemical.   On or about October 7, 2021, at an establishment located at 1100 East Green Bay Street in Shawano, Wisconsin, in the Rig Room;  the employer did not ensure that a plastic bottle of engine stabilizer was labeled, tagged, or marked to provide at least general information regarding the hazards of the chemicals.  Employees were exposed to eye and skin irritation when using a bottle of STA-BIL engine stabilizer to winterize boat motors.

Chemical-specific information must always be available through labels and safety data sheets.   The employer did not provide employees with chemical specific information and training on hazardous chemicals in their work area.
