29 CFR 1926.451(c)(1)(ii): Guys, ties, and/or braces were not installed according to the scaffold manufacturer's recommendations or at the closest horizontal member to the 4:1 height and repeated vertically at locations of horizontal members every 20 feet (56.1 m) or less thereafter for scaffolds 3 feet ((0.91 m) wide or less, and every 26 feet (7.9 m) or less thereafter for scaffolds greater than 3 feet (0.91m) wide: On September 28th, 2021 at the jobsite located at 1400 Baltimore Ave in Kanas City, Missouri employees were exposed to a fall hazard. The Hydro Mobile brand S series mast climber the employees were working from tipped over from an approximate height of 25 feet. The mast climber tipped when it was not stabilized during dismantling according to engineering drawings for the mast climber.

29 CFR 1926.454(b)(2): The employer did not have each employee who was involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the correct procedures for erecting, disassembling, moving, operating, repairing, inspecting, and maintaining the type of scaffold in question: The employer is failing to protect employees by failing to instruct employees in the recognition and avoidance of unsafe conditions such as, but not limited to fall hazards. This was most recently documented on September 28, 2021, at a worksite located at 1400 Baltimore Avenue, Kansas City, Missouri. Two employees were injured when the Hydro Mobile brand mast climber the employees were working from fell to the ground while the employees were attempting to dismantle it. The employer did not adequately train the employees in the steps necessary to safely dismantle the mast climber.
