The employer did not have a ladder training program in place for employees who were working off ladders while installing roof trusses.

29 CFR 1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a) On or about 9/21/2021 at 909 Skyline Blvd, Cape Coral FL, 33991, two employees suffered work-related injuries and were hospitalized. The employer was aware of the in-patient hospitalization on 9/21/2021. The employer notified OSHA on 9/23/21 at 12:20 PM of the in-patient hospitalizations.
