Location - Domiciliary unit (DOM) at 200 Spring Rd., Bedford, MA, 01730 On or about September 2, 2021, the employer became aware that an employee tested positive for COVID-19. Co-workers who worked with this employee wore surgical masks and were not provided N95 respirators. Six employees tested positive resulting in two hospitalizations one of which resulted in the death of an employee.

Location - Domiciliary unit (DOM) at 200 Spring Rd., Bedford, MA, 01730 Employer did not notify employees who were in close contact at the domiciliary (DOM) unit to a person who is COVID-19 positive. On or about September 2, 2021, Employee 1 was identified as a close contact to a co-worker who tested positive for COVID-19 on that same day September 2, 2021. Employee 1 was not wearing a respirator. The employer did not notify Employee 1 within 24 hours. Employee 1 reported to work on September 4th and remained at work until he received a positive test result.

The employer was notified on September 19, 2021 of the hospitalization. The employer did not report this hospitalization to OSHA.
