29 CFR  1926.503(a)(1):The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.  117 Putnam Hill Rd, Sutton, MA:  On October 7, 2021, the employer had not trained the five employees who were exposed to fall hazards, while performing roofing work, to recognize fall hazards and on the procedures to be followed to minimize these hazards.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   117 Putnam Hill Rd, Sutton, MA:  On October 7, 2021, employees were not protected from fall hazards while performing roofing work on a residential roof with the roof edge more than ten feet above the ground.  The foreman employee was not wearing a personal fall arrest system while working on the roof and subsequently fell to the ground and received a shoulder injury.  An additional employee was observed dismounting a ladder and walking approximately ten feet along the roof ridge prior to connecting the lanyard to the harness.  Active Pro Construction Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1455345, citation number 1, item number 1, and was affirmed as a final order on February 10, 2020, with respect to a workplace located at 1272 Brook Rd, Milton, MA. Active Pro Construction Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1431577, citation number 1, item number 1, and was affirmed as a final order on December 5, 2019, with respect to a workplace located at 80 Lowell St, Lexington, MA. Active Pro Construction Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1368208, citation number 1, item number 3A and was affirmed as a final order on  April 23, 2019, with respect to a workplace located at 17 Tremont St, Braintree, MA.
