Location: Job site; On or about 09/13/2024, employees were observed working in the zone of danger beneath an active roofing project where roofing materials and equipment were not secured or protected from falling and striking workers in the head on the ground who were not protected by hardhats.

29 CFR 1926.502(d)(15): Anchorage points used for attachment of personal fall arrest equipment were not independent of any anchorage point being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee. Location: Job site; On or about 09/13/2024, employees were allowed to utilize temporary roof anchors which were not installed per manufacturer instructions to support 5,000lbs per employee. Anchorage points were installed along the peak of the roof running in-line with the peak instead of perpendicularly and anchored to the roof decking.

Location: Job site; On or about 09/13/2024, the employer allowed employees to engage in roofing activities without the use of fall protection on a residential roof that was 23 to 25-feet above the ground below. Active Pro Construction Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1556848, citation number 2, item number 1 and was affirmed as a final order on 02/07/2022, with respect to a workplace located at 117 Putnam Hill Rd, Sutton, MA 01590.
