29 CFR  1910.22(a)(1):(1) All places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition.  On or about October 6, 2021, the employer did not ensure the walking-working surface of tie machine department/room was kept in a clean, orderly, and sanitary condition.  Boxes, finished parts, oil dry, pallets, and other materials were cluttered on the floors exposing employees to slip, trip, and fall injures and limited the access and egress of aisle ways in the department/room.

29 CFR  1910.36(g)(2):An exit access must be at least 28 inches (71.1 cm) wide at all points. Where there is only one exit access leading to an exit or exit discharge, the width of the exit and exit discharge must be at least equal to the width of the exit access.  On or about October 6, 2021, the employer did not ensure the access to the exit door from the tie machine department room was at least 28 inches wide.  Material stored in front of the exit door limited the size of the access to approximately less than 24 inches.

29 CFR  1910.147(c)(1):Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.  On or about October 6, 2021, the employer did not establish an energy control program consisting of energy control procedures, employee training, and periodic inspections.  Employees were not protected from the unexpected energization, startup, or release of stored energy while performing servicing and maintenance activities, such as, but not limited to, setup operations, machine adjustments, machine repairs, and clearing jams on the wire tie machines, wire weaving machines and/or wire drawing machines.  Energy sources include, but not limited to, electrical and hydraulic energy.

29 CFR  1910.212(a)(1):Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  a.	On or about October 6, 2021, the employer did not ensure operator(s) of the wire tie machines were protected by one or more methods of machine guarding from the moving and rotating parts of the puller and belts and pulleys; ingoing nip points of the tension rollers; and point of operation of the wire twister and cutter.   b.	On or about October 6, 2021, the employer did not ensure operator(s) of the wire weaving machines were protected by one or more methods of machine guarding from the moving and rotating parts of the cutter; ingoing nip points of the tension rollers; and point of operation of the rotating saddle and weaving blade.  c.	On or about October 6, 2021, the employer did not ensure operator(s) of the wire drawing machines were protected by one or methods of machine guarding from the ingoing nip points created by the wire moving of the wire over the pulleys and wire drawing capstans and the moving and rotating parts of the pulleys and capstans.

29 CFR  1910.305(b)(2)(i):All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.  On or about October 6, 2021, the employer did not ensure a cover or faceplate was installed over a 110 volt outlet box in the tie machine department.
