29 CFR  1910.253(b)(4)(i): Oxygen cylinders were stored near highly combustible material, especially oil and grease; or near reserve stocks of carbide and acetylene or other fuel-gas cylinders, or near other substance likely to cause or accelerate fire:   On or about 9/7/2023, at the work location of 1600 Firestone Parkway, Akron Ohio 44301 the employer failed to ensure that oxygen tanks were stored away from acetylene tanks. In the Common area (break room), walkway (pedestrian walk through), and path of travel for PIVs (along the wall in the passage way between warehouse and welding/fabrication departments) the oxygen and acetylene tanks were stored together standing along a concrete block wall, and were only used about once a week.

On or about September 7, 2023, the employer did not ensure operator(s) of the #1 Bergandi Weaving Machine were protected by one or more methods of machine guarding from the moving and rotating parts of the feeding arm pulleys; ingoing nip points of the tension rollers; of the weaving machine feeding arm (9 inches wide, 12 inches long, 3 inch open channel holding pulleys) as employees reach over, around, or through the area to operate the machines controls located 15 inches behind the feed arm. Employees are also exposed while leaning over to checking, adjusting, and collecting the finished product at the finished collection system along the drive side of the machine due to unguarded gears, sprockets, and drive chains (28 inches in depth, approx., and power of the rolling action.   b. On or about September 7, 2023, the employer did not ensure operator(s) of the #4 Wire Drawing Machine (Located in the wire Department) were protected by one or more methods of machine guarding from the moving and rotating parts of the feeding arm pulleys; ingoing nip points of the tension rollers; of the wire drawing machine feeding/drawing metal wire/material into and or through the pulleys and or guides at a rate of 90 feet per minute.   k.	On or about September 7, 2023, the employer did not ensure operator(s) of the #3 Wire Drawing Machine (Located in the wire Department) were protected by one or more methods of machine guarding from the moving and rotating parts of the feeding arm pulleys; ingoing nip points of the tension rollers; of the wire drawing machine feeding/drawing metal wire/material into and or through the pulleys and or guides at a rate of 90 feet per minute.   l.	On or about September 7, 2023, the employer did not ensure operator(s) of the #2 Wire Drawing Machine (Located in the wire Department) were protected by one or more methods of machine guarding from the moving and rotating parts of the feeding arm pulleys; ingoing nip points of the tension rollers; of the wire drawing machine feeding/drawing metal wire/material into and or through the pulleys and or guides at a rate of 90 feet per minute.   Richards Wholesale Fence Co Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1910.212(a)(1) Machine Guarding), which was contained in OSHA inspection number 1556863, citation number 01, item number 03 and was affirmed as a final order on (1/27/2022), with respect to a workplace located at 1600 Firestone Parkway, Akron Ohio 44301.
