29 CFR  1926.451(b)(1):Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii).  On or about October 4, 2021 and times prior there to, the employer did not ensure that all working levels of scaffold were fully planked on the first and second tier of the tubular welded scaffold system in the following instances:  a) first tier, middle section where there was a piece of plywood covering the 10 inch gap in the planking b) second tier, the left and middle section were not fully planked from guard rail support to guard rail support

29 CFR  1926.451(b)(3):Except as provided in paragraphs (b)(3)(i) and (ii) of this section, the front edge of all platforms shall not be more than 14 inches (36 cm) from the face of the work, unless guardrail systems are erected along the front edge and/or personal fall arrest systems are used in accordance with paragraph (g) of this section to protect employees from falling.  On or about October 4, 2021 and times prior there to, the employer did not ensure the welded tubular scaffold system was erected in such a manner to have the front edge of the working platform to be no more than 14 inches from the face of the work zone.

On or about October 4, 2021 and times prior there to, the employer did not ensure the footings of the tubular welded scaffold system were level and not subjected to settling caused by standing water or displacement in the following instances:  a) Far left section of scaffold, footing not secure or square on wooden base plate b) Right section of scaffold to right of doorway, footings of scaffold were sinking into mud due to standing water

29 CFR  1926.451(c)(3): Supported scaffold poles, legs, post, frames, and/or uprights were not plumb and braced to prevent swaying and displacement.  On or about October 4, 2021 and times prior there to, the employer did not ensure the scaffold system erected on the side of the house was supported and braced in a way to prevent swaying while employees occupied scaffold system.

29 CFR  1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  On or about October 4, 2021 and times prior there to, the employer did not ensure employees tasked with accessing a multi-tiered scaffold system were provided with a safe means of accessing the scaffold via ladder or direct access.
