29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest. On or about October 8, 2021. Employees were conducting roofing work on the west side of the site heights greater than six (6) feet without guard rails or fall protection working.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. On or about October 8, 2021, on the south side of the construction site, employees were utilizing a Werner extension ladder that was not extended three (3) feet above the upper landing surface and not secured at any point.
