On or about November 8, 2021, employees conducted roofing/flashing work on the north side of the site at heights greater than six (6) feet without guardrails or fall protection.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access. On or about November 8, 2021, employees on the south side of the site performing flashing work were using a ladder to gain access to the roof area where the ladder was not extended three (3) feet above the upper landing surface and not secured at any point.
