29 CFR 1926.501(b)(4)(i):Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. a) 2944 Samuel Dr., Bensalem, Pa, Warehouse - On or about October 11, 2021, employees were engaged in roofing activities that included removal of roof ventilators. The removal of roof ventilators created an unprotected hole in the metal roof deck, exposing employees to a fall hazard of approximately 32' to the finished concrete floor below. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photograph or video evidence of abatement or other written records.

29 CFR 1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: a) 2944 Samuel Dr., Bensalem, Pa, Warehouse - On or about October 11, 2021, employees were engaged in roofing activities that included removal of roof ventilators. The employer did not establish a program that adequately trained employees to recognize fall hazards. Abatement certification and documentation required within 10 days after abatement date.
