29 CFR 1910.333(a)(2): Where exposed live parts were not deenergized (i.e., for reasons of increased or additional hazards or infeasibility), other safety-related work practices were not used to protect employees who could be exposed to the electrical hazards involved: a) Jobsite, 31 Union Electric Road, Burgettstown, PA 15021: On or about September 22, 2021, employees were exposed to electrical shock hazards when exposed to energized conductors (480 volt, 3 phase) located inside of the control panels of the lathes including the #24 tailstock lathe. Safety-related work practices were not developed or used to protect employees from electrical shock hazards while preforming various types of troubleshooting activities on the lathes including, but not limited to, checking the quill, checking amperages and checking inputs/outputs to different relays.

29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not using electrical protective equipment that was appropriate for the specific parts of the body to be protected and for the work to be performed: a) Jobsite, 31 Union Electric Road, Burgettstown, PA 15021: On or about September 22, 2021, employees engaged in troubleshooting the lathes, including lathe #24, where not using proper personal protective equipment during troubleshooting activities. Employees use Tilsatec brand TTP203 leather gloves while working inside of the lathe control panels which contain energized conductors (480 Volts). The gloves do not offer an adequate level of protection to the employee's hands and arms when they are required to troubleshoot components inside of the lathe control boxes.
