29 CFR  1910.147(c)(7)(i):The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices:    On or about October 13, 2021, throughout the facility, employees were exposed to caught-in, struck-by and electrical hazards while servicing and performing maintenance work on various wood mill machinery without being adequately trained on the safe application of energy control devices.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    On or about October 13, 2021, in the Dry Mill area of the facility, employees were exposed to caught-in and struck-by hazards while working on the dry mills without following established energy control procedures.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  On or about October 13, 2021, in the ash conveyor area, employees were exposed to caught-by hazards while working near the ash conveyor mixing paddle without a guard in place to cover the rotating paddle.

On the northwest side of the wood mill, employees were exposed to a caught-by hazard while working near the bark belt without guards to cover belt pulleys.  b. On the west side of the wood mill, employees were exposed to a caught-by hazard while working near the bark stacker without a guard cover on the belt tail pulley.  c. On the west side of the wood mill where employees were exposed to a caught-by hazard while working near and walking under the Veco belt without guards to cover belt pulleys.

29 CFR 1910.179(j)(3): Complete inspections of the crane was not being performed at intervals as generally defined in paragraph (j)(1)(ii)(b) of this section:  On or about October 13, 2021, and at times prior thereto, on the north side of the wood mill, employees were using a Price Logpro 175 foot, 25 ton radial log crane SN 1204-02-1 that had not been inspected at least annually.
