29 CFR 1926.501(b)(10):Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system: a) Northwest section of roof, on or about 10-14-21: Employees were working on a roof and were not protected from falls, exposing the employees to a fall hazard of 19 feet 5 inches. b) Northeast section of roof, on or about 10-14-21: Employees were working on a roof and were not protected from falls, exposing the employees to a fall hazard of 16 feet 6 inches. c) Southwest section of roof, on or about 10-14-21: Employees were working on a roof and were not protected from falls, exposing the employees to a fall hazard of 12 feet 2 inches.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) Southwest section of building, east side, on or about 10-14-21: Twelve employees used one of two extension ladders for access to and egress from a roof that were not extended 3 feet above the landing surface, in that each was extended 27" above the roof surface, exposing the employees to a fall hazards of 12 feet 2 inches.
