29 CFR  1910.95(d)(1):When information indicated that any employee's exposure equaled or exceed the 8-hour time-weighted average of 85 decibels, the employer did not develop and implement a monitoring program:  On or about October 20, 2021, and at times prior thereto, the employer did not develop and implement a noise monitoring program.   a. An employee inspecting plastic materials from the injection molding machine in the blow room was exposed to continuous noise during the 424-minute sampling period at an 8-hour TWA sound level of 89 dB (92% of the permissible daily dose).  b. An employee inspecting and packaging plastic materials from the bin at the injection molding machine in the blow room was exposed to continuous noise during the 440-minute sampling period at an 8-hour TWA sound level of 90 dB (98% of the permissible daily dose).

29 CFR  1910.95(g)(1):The employer did not establish and maintain an audiometric testing program as provided by 29 CFR 1910.95(g) by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels:  On or about October 20, 2021, and at times prior thereto, the employer did not establish and maintain an audiometric testing program that included audiometric testing of employees and retention of exposure measurements.  a. An employee inspecting plastic materials from the injection molding machine in the blow room was exposed to continuous noise during the 424-minute sampling period at an 8-hour TWA sound level of 89 dB (92% of the permissible daily dose).  b. An employee inspecting and packaging plastic materials from the bin at the injection molding machine in the blow room was exposed to continuous noise during the 440-minute sampling period at an 8-hour TWA sound level of 90 dB (98% of the permissible daily dose).

29 CFR  1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  On or about October 15, 2021, and at times prior thereto, at the blow room, employees were exposed to injurious corrosive material when performing cleaning activities without having access to emergency flushing equipment.
