29 CFR  1910.22(c):(c) Access and egress.  The employer must provide, and ensure each employee uses, a safe means of access and egress to and from walking-working surfaces:  (a)  BACK WAREHOUSE, TANK AREA - Employees used the top step and the cap of a Werner eight foot step ladder to access the top of the salt tank and then manually climbed approximately 36 inches to an adjacent tank which was 11 feet high without any additional equipment to facilitate safe access to the tanks, on or about October 13, 2021.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

Abatement certification required within 10 days after abatement date. 29 CFR 1910.28(b)(1)(i):  Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  (a)  BACK WAREHOUSE, TOPS OF CHEMICAL HOLDING TANKS - Employees walked across and worked from the tops of the salt tank, the re-use water tank, and the evaporator salts tank when servicing the tank equipment and the employer had not ensured that the employees were protected from falling , on or about October 13, 2021.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

1910.132(f)(1)(iv):  Employee(s) required to use PPE by this section were not trained to know the limitations of PPE:   (a)  PRODUCTION AREA - Employees performed electroplating operations where the work involved placing parts into and removing parts from tanks that were filled with chemicals that included strong acids with PH ratings less than 6 and strong alkalines with PH ratings of greater than 12.    The employer did not require and enforce the use of personal protective required by safety data sheets for the chemicals that were being used which included safety goggles and/or face shields, on or about October 13, 2021.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
