Located at the Hampton Inn at 2041 Schorrway Drive, Lancaster, Ohio: On or about October 14, 2021, and at times prior, the employer did not initiate and maintain safety and health programs to ensure that hazards were identified and corrected onsite while employees performed commercial exterior remodeling and EFAS activities. As a result, employees were exposed to hazards including, but not limited to, electrical, falls, and struck-by, associated with their job tasks.

Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers.  a. As a result, employees were working from unsafe supported scaffolds that had not been inspected and were thereby exposed to fall hazards of up to 35 feet.

Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width (see Appendix A to subpart M of this part), the use of a safety monitoring system alone [i.e. without the warning line system] is permitted.  a. Located at the Hampton Inn at 2041 Schorrway Drive, Lancaster, Ohio: On or about October 14, 2021, an employee was working at an unprotected edge of the low-sloped roof without any fall protection measures in place. As a result, the employee was exposed to a fall hazard of 18 feet up to 35 feet.

Located at the Hampton Inn at 2041 Schorrway Drive, Lancaster, Ohio: On or about October 14, 2021, an un-trained employee was working from the unprotected edge of the roof exposed to fall hazards without utilizing any fall protection measures in use. The employer did not ensure that the employee was trained on the fall hazards and procedures and methods to minimize the fall hazards. As a result, the employee was exposed to fall hazards of approximately 35 feet.
