This was most recently documented on or about October 14th, 2021 in Elkhorn, Nebraska. The unprotected stairwell was used by multiple employees for access and egress from the main floor landing area to the 2nd floor and it was not equipped with a stair rail system along each protected side or edge - the measurement from the lower level to the top landing area of the 2nd floor was over 10 feet.

29 CFR  1926.1053(b)(4):Ladder(s) were used for purposes other than the purpose for which they were designed:  The employer is failing to protect employees from fall hazards associated with improper use of  "A" frame ladders.  This was most recently documented on or about October 14th, 2021 in Elkhorn, Nebraska.  Employees were using an "A" frame ladder for access/egress to the roof.  In this particular scenario, the A frame ladder was not opened/locked according to the manufactures requirement - The ladder was stowed in the close position and leaning up against the side of the home.

An employee was engaged in residential framing activities where the employee observed was working on a scaffolding and the planks of the scaffolding was not at least 18 inches wide. This incident was most recently documented on or about October 14th, 2021, in Elkhorn, Nebraska.  Abraham Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(b)(2), which was contained in OSHA inspection number 1548397, citation number 1, item number 3(a) and was affirmed as a final order on 9-29-21, with respect to a workplace located at 2702 Hildy Street, ELKHORN, NE 68022.

29 CFR  1926.451(b)(3):The front edge of all platforms was not 14 inches (36 cm) or less from the face of the work, and no guardrail systems were erected along the front edge and/or no personal fall arrest systems were used in accordance with paragraph (g) of this section to protect employees from falling.  The employer is failing to protect employees to potential fall hazards. An employee was observed engaged in residential framing activities where the face of the work being performed was greater than 14 inches. This was most recently documented on or about October 14th, 2021 in Elkhorn, Nebraska. The exposed employee was located on top of carpenters scaffolding conducting framing operations on the exterior perimeter of the residential structure.  Abraham Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(b)(2), which was contained in OSHA inspection number 1548397, citation number 1, item number 3(b) and was affirmed as a final order on 9-29-21, with respect to a workplace located at 2702 Hildy Street, ELKHORN, NE 68022.

Multiple employees were observed working on a residential structure conducting framing operations. The exposed employees were working on the roof of the residential structure at heights over 12 feet- No fall protection was being utilized.
