Location: a) Mobile scaffold on the second floor level of the jobsite: On or about October 19, 2021, employees were working on the drywall on an approximately 6 feet tall by 2 feet wide mobile scaffold. The scaffold wheels were not locked to prevent movement of the scaffold.
