Capacity, operation, and maintenance instruction plates, tags, and decals were not changed accordingly: Fabrication Shop, Hyster forklift model H80XM, Serial number L005V02943B - On or about October 20, 2021, modifications and additions were made to the forklift without permission from the manufacturer and data tags or decals were not changed accordingly. The forklift was equipped with a G-Force Fabrication Inc. boom extension marked as serial number 090404 (utilized at approximately 9 feet) and slab clamp that affected the capacity and safe operation while it was used to lift and transport stone slabs.

Fabrication Shop, Hyster forklift model H80XM, Serial number L005V02943B - On or about October 20, 2021, the G-Force Fabrication Inc. boom extension, serial number 090404, was used to lift, transport, and lower stone slab loads while the boom was not attached to the forklift with it's securing chain. Additionally, the load end of the boom was excessively worn where the slab lifting clamp was attached.
