Capacity, operation, and maintenance instruction plates, tags, and decals were not changed accordingly:      Fabrication Shop, Hyster forklift model H80XM, Serial number L005V02943B - On or about October 20, 2021, modifications and additions were made to the forklift without permission from the manufacturer and data tags or decals were not changed accordingly. The forklift was equipped with a G-Force Fabrication Inc. boom extension marked as serial number 090404 (utilized at approximately 9 feet) and slab clamp that affected the capacity and safe operation while it was used to lift and transport stone slabs.

29 CFR  1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s).  Fabrication Shop, Hyster forklift model H80XM, Serial number L005V02943B - On or about October 20, 2021,  the G-Force Fabrication Inc. boom extension, serial number 090404, was used to lift, transport, and lower stone slab loads while the boom was not attached to the forklift with it's securing chain.  Additionally, the load end of the boom was excessively worn where the slab lifting clamp was attached.

Fabrication shop -  On or about October 20, 2021, access to the electrical disconnect was not provided and maintained for the Fusion 4045 CNC slab saw to permit ready and safe operation where there was plate steel stacked on the floor of the pathway and impeding access to it.
