29 CFR 1910.28(b)(11)(ii): The employer did not ensure that each flight of stairs having at least 3 treads and at least 4 risers is equipped with stair rail systems and handrails as described in 29 CFR 1910.28 Table D-2-Stairway Handrail Requirements. South receiving dock: The staircase from the dock to the ground had six treads and seven risers, and did not have a stair rail system and handrail on the open side. Employees are exposed to injuries from falling.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries: South Receiving Area, near entrance to main floor: The fire extinguisher mounted on the wall was blocked from access due to food and a garbage can were in front of the fire extinguisher. This condition exposed the employees to hazards associated with fire such as burns and smoke inhalation.

29 CFR 1910.303(g)(1)(i):The dimension of the working space in the direction of access to live parts operating at 600 volts or less and likely to require examination, adjustment, servicing, or maintenance while alive was less than indicated in Table S1: (a) West Receiving Area, near food walk-in refrigerator: The employer stored portable step ladders in front of an electrical panel blocking access to the panel. Employees exposed to electrical hazards. (b) South Receiving Area, near entrance to main floor: The employer stored racks, shopping cart, and food containers in front of electrical panels blocking access to the panels. Employees exposed to electrical hazards. The Defense Commissary Agency was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.303(g)(1), which was contained in OSHA inspection number 1232772, citation number 2, item number 1 and was affirmed as a final order on 8/8/17, with respect to a workplace located 17601 25th Street Beale AFB, CA 95903.
