29 CFR  1926.100(a):Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  On or about October 20, 2021, employees were exposed to a struck by hazard while performing siding activities on a residential home located at 27893 Ironwood Court, Westlake, Ohio 44145.  Employees were working on upper levels with tools and materials that could have fallen on lower employees and the employer did not ensure that the lower employees were wearing protective helmets.

29 CFR  1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.    On or about October 20, 2021, employees were exposed to a fall of approximately 13 feet while performing siding activities on a residential home located at 27893 Ironwood Court, Westlake, Ohio 44145.  An employee was working on a ladder pick scaffold with the ladders on the inside of the picks and the employer did not ensure that he had a safe access point to the platform.

An employee was working on a ladder pick scaffold  and the employer did not ensure that he was using fall protection.

An employee was working on the roof of the home and the employer did not ensure that he was using fall protection.

An employee was working on the roof of the home and the employer did not ensure that he was trained on how to recognize and minimize fall hazards.
