29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  At 638 Mendelssohn Dr, Kirkwood, MO 63122 - On or about October 19, 2021 employees of JSG General Contracting LLC, were exposed to the hazard of being struck by flying objects from using a pneumatic nail gun while not wearing eye protection.

Each employee who was engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system.  At 638 Mendehlssohn, Kirkwood, MO 63122 - On or about October 19, 2021 Employees of JSG General Contracting LLC were exposed to elevated fall hazards over six feet while  working on the roof of a residential structure and were not protected by a guardrail system, safety net system or personal fall arrest system.

29 CFR 1926.1053(b)(1): Portable ladders were used to access an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access.  At 638 Mendelssohn Dr, Kirkwood, MO 63122 - On or about October 19,2021 employees of JSG General Contracting LLC were exposed to possible fall hazards from a ladder used to access an upper landing surface which did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access.
