29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  On or about July 16, 2021, on the expansion mezzanine of the establishment located at 8501 U.S. Highway 27 in South Bay, Florida, employees were exposed to amputation hazards when changing a tote bag on a dust collector where the employer had not developed, documented, and utilized a procedure to control potential hazardous energy for that machine.

29 CFR  1910.147(c)(7)(i)(B): Affected employees were not instructed in the purpose and use of the energy control procedure:   On or about July 16, 2021, on the expansion mezzanine of the establishment located at 8501 U.S. Highway 27 in South Bay, Florida, an affected employee was exposed to amputation hazards when changing a tote bag on a dust collector prior to being provided training that would enable him recognize the hazards associated with the lack of energy control procedures.
