29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  On or about 10/22/2021, in the packaging department blending room of the establishment located at 8501 U.S. Highway 27 in South Bay, Florida, an employee was exposed to amputation hazards when manually rotating a pully to perform welding work on a metering screw where the employer had not developed, documented and utilized a procedure to control potential hazardous energy for that machine.

29 CFR  1910.147(c)(7)(i)(B): Affected employees were not instructed in the purpose and use of the energy control procedure:   On or about 10/22/2021, in the packaging department blending electrical room of the establishment located at 8501 U.S. Highway 27 in South Bay, Florida, an authorized employee applied Lockout/Tagout devices for a metering screw prior to being provided specific training on the equipment that would enable him to recognize the correct circuits shut offs when implementing and/or using energy control procedures.
