On or about October 20, 2021, at the establishment located at N3151 State Road 47 Appleton, WI 54913, the plug on the Sears Craftsman 12-inch electric band saw was missing the ground pin which exposed employees to potential electric hazard. b. On or about October 20, 2021, at the establishment located at N3151 State Road 47 Appleton, WI 54913, the plug on the Ventimatic BF24TF YEL MaxxAir 24-inch floor fan was missing the ground pin which exposed employees to potential electric hazard. c. On or about October 20, 2021, at the establishment located at N3151 State Road 47 Appleton, WI 54913, the plug on the Patton Electric Industrial Floor Fan was missing the ground pin which exposed employees to potential electric hazard.

29 CFR 1910.305(b)(1)(ii):Unused openings in cabinets, boxes, and fittings shall be effectively closed. a. On or about October 20, 2021, at the establishment located at N3151 State Road 47 Appleton, WI 54913, an electric junction box was not effectively closed. The junction box, located on the edge bander at the southwestern corner of the production area, was missing a knockout. b. On or about October 20, 2021, at the establishment located at N3151 State Road 47 Appleton, WI 54913, an electric junction box was not effectively closed. The junction box, on the western wall of the production area, had an opening on the top where a piece of conduit was removed. c. On or about October 20, 2021, at the establishment located at N3151 State Road 47 Appleton, WI 54913, an electric cabinet was not effectively closed. The electric cabinet in the (old break room) had an opening in the breaker area and was missing two breakers along with a missing cover on the bottom of the cabinet.

29 CFR 1910.334(a)(2)(ii):If there is a defect or evidence of damage that might expose an employee to injury, the defective or damaged item shall be removed from service, and no employee may use it until repairs and tests necessary to render the equipment safe have been made. a) On or about October 20, 2021, employees were exposed to potential shock hazards when working with the Sears Craftsman 12-inch electric band saw. The power cord had exposed wires at the base of the plug and employees were exposed to electric shock when plugging in or unplugging the power cord. b) On or about October 20, 2021, employees were exposed to potential shock hazards when working with a Southworth 2000-pound capacity lift table. The power cable had exposed wires at the base of the plug and employees were exposed to electric shock when plugging in or unplugging the power cord.

In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear. On or about October 20, 2021, at the establishment located at N3151 State Road 47 Appleton, WI, employees were exposed to electrical hazards from a junction box missing a cover.
