(a) On or about October 20, 2021 at the job site located at W194N4976 Hickory St, Menomonee Falls WI 53051 an employee was not wearing eye protection while using a pnuematic nail gun to install shingles on a roof.

(a) On or about October 20, 2021, at the jobsite located at W194N4976 Hickory Street, Menomonee Falls WI, employees were  performing roofing work on a two story multi family apartment unit with a pitch of 8:12 and up to approximately 9 8/10 feet without any fall protection in place, thereby exposing the employees to a fall hazard.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   (a) On or about October 20, 2021, at the jobsite located at W194N4976 Hickory Street, Menomonee Falls WI, employees were  performing roofing work on a two story multi family apartment unit with a pitch of 8:12 and up to approximately 9 8/10 feet without any fall protection in place, thereby exposing the employees to a fall hazard. Employees did not have fall protection training.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) On or about October 20, 2021, at the jobsite located at W194N4976 Hickory Street, Menomonee Falls WI, employees were  performing roofing work on a two story multi family apartment unit were using a ladder which was not three feet above the lading surface on the roof.
