29 CFR  1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   (a) On or about July 13 2022, at the jobsite located at N168W21971 Main Street, Jackson WI, an employee was performing clean up on the ground without a hard hat on while other employees were installing shingles on the roof above.

29 CFR  1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  (a) On or about July 13 2022, at the jobsite located at N168W21971 Main Street, Jackson WI, employees were  performing roofing work on a single story commercial building with a pitch of 6:12 and up to approximately 11 1/10 feet without any fall protection in place, thereby exposing the employees to a fall hazard.  The Padilla Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 1559567, citation number 1, item number 2a and was affirmed as a final order on 5/18/22, with respect to a workplace located at W194N4976 Hickory Street, Menomonee Falls WI.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   (a) On or about July 13 2022, at the jobsite located at N168W21971 Main Street, Jackson WI, employees were  performing roofing work on a single story commercial building with a pitch of 6:12 and up to approximately 11 1/10 feet without any fall protection in place, thereby exposing the employees to a fall hazard. Employees did not have fall protection training.  The Padilla Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(a)(1), which was contained in OSHA inspection number 1559567, citation number 1, item number 2b and was affirmed as a final order on 5/18/22, with respect to a workplace located at W194N4976 Hickory Street, Menomonee Falls WI.
