29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and/or walkway was not at least 18 inches (46 cm) wide: At a worksite located at 4455 E Mustard Way, Springfield, MO 65803: Employees were exposed to fall hazards. Employees were observed exiting the guardrails of the platform basket and using the railing to gain access to the roof. Employees were exposed to heights of 15 feet 6 inches to the ground below. Employees were not protected from falls by the use of guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems. Such exposure is likely to cause serious physical harm or death. This was last demonstrated on October 22, 2021.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems: At a worksite located at 4455 E Mustard Way, Springfield, MO 65803: Employees were exposed to fall hazards. Employees were observed working at heights next to an unprotected side or edge while erecting a new structure. Employees were exposed to heights of 15 feet 6 inches to the ground below. Employees were not protected from falls by the use of guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems. Such exposure is likely to cause serious physical harm or death to occur. This was last demonstrated on October 22, 2021. The Burk Erectors Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (29 CFR 1926.760(a)(1)), which was contained in OSHA inspection number 1386281, citation number 1, item number 1 and was affirmed as a final order on (April 19, 2019), with respect to a workplace located at 30852 Hwy 181, Spanish Fort, AL 36527.
