29 CFR 1926.20(b)(2):The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person (i.e., a person who is capable of identifying existing hazards) Location: Jobsite Two workers were injured, and more workers were exposed to fall hazards, where a competent person did not adequately inspect to ensure that the walking working surface was adequate to support the load imposed by them.

29 CFR 1926.501(a)(2):The employer shall determine if the walking/working surfaces on which its employees are to work have the strength and structural integrity to support employees safely. Employees shall be allowed to work on those surfaces only when the surfaces have the requisite strength and structural integrity. Location: Column R43, 31st Floor, 133 Federal Street, Boston, MA Two employees setting a rebar column fell through the walking and working surface, the concrete form work, to the floor below.
