29 CFR 1910.22(a)(1): The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition: (a) Golden Aluminum, Inc. at 1405 E. 14th Street, Fort Lupton, CO 80621: On and before October, 25, 2021, the employer did not ensure that all walking working surfaces were kept in a clean, orderly, and sanitary condition, in that the walk-working surfaces immediately next to the Cold Mill machine were not kept free from excess oil that the Cold Mill machine releases during operations. Employees are required to use to the walking-working surface on a daily basis to change the rollers on the Cold Mill machine. This condition exposes employees to injuries associated with slips, trips, and falls.

29 CFR 1910.147(a)(2)(ii): Alternative measures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by the exception of 1910.147(a)(2)(ii) for routine, repetitive, and integral processes such as minor tool changes and adjustments, and other minor servicing activities. (a) Golden Aluminum, Inc. at 1405 E. 14th Street, Fort Lupton, CO 80621: On and before October 25, 2021 the employer did not ensure that alternative measures were developed, documented and utilized for the control of potentially hazardous energy when employees were required to engage in tool changes of the Cold Mill machine. Employees were required to be positioned in an associated danger zone that exists during machine operation in order to complete the tool change of the Cold Mill Machine. This condition exposed employees to nip points and crushed-by hazards. (b) Golden Aluminum, Inc. at 1405 E. 14th Street, Fort Lupton, CO 80621: On October 16, 2021, the employer did not ensure that alternative measures were developed, documented and utilized for the control of potentially hazardous energy when employees were required to engage in tool changes of the Cold Mill machine. An employee who was engaged in the tool change of the Cold Mill machine sustained a partial amputation of three fingers on his left hand.
