29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) On October 16, 2021, an employee was installing a dormer fascia on the roof truss of a single-story residential house. The employee was not protected by a fall protection system when he fell approximately 10 feet to the ground. The employee suffered multiple broken bones and was admitted to the hospital.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: a) On October 16, 2021, an employee fell approximately 10 feet to the ground and suffered multiple broken bones and was admitted to the hospital. The employer reported the incident on October, 26, 2021, 10 days after the incident occurred and not within the 24-hour reporting period.
