On or about October 26, 2021, Assemblers were using an alloy steel chain to hoist motor that did not have an identification tag that indicated size, grade, rated capacity, and reach of the chain.

29 CFR 1910.184(e)(2)(ii): Makeshift links or fasteners formed from bolts or rods, or other such attachments, shall not be used. On or about October 26, 2021, employees were using climbing karabiners to attach an alloy steel chain to the hook of a Pittsburgh Heavy Duty 1 Ton Folding Shop Crane and to the motors that are being lifted by the chain sling.
