29 CFR 1910.95(g)(5)(i): The employer did not establish, within 6 months of an employee's first exposure at or above the action level, a valid baseline audiogram against which subsequent audiograms can be compared: a) On or about October 27, 2021 at the 1632 Third Street, Leesburg, FL 34748 employees performing production activities were exposed to noise levels at 71.45-151.1% of the allowable 8-hour Time Weighted Average (85 dBA), with an equivalent dBA level of 87.5-92.9 dBA. The employer failed to require a baseline audiogram within six months of the employee's first exposure above the action level.
