Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measures. On or about 10/21/2021, employees from RDZ Exteriors were removing roofing material from a roof, approximately 9 feet from ground level, of a residential building located at 7608 Tennessee Ave, St. Louis, MO 63111. The employees were not protected by guardrails systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measures. The workers were exposed to a fall hazard.

29 CFR 1926.1053(b)(1):When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access. On or about 10/21/2021, workers from RDZ Exteriors were removing shingles from a residential roof located at 7608 Tennessee, St. Louis, MO. The general contractor (Over the Top Roofing), the sub-contractor (RDZ Exteriors), were using an extension ladder to access the roof. The aluminum extension ladder side rails did not extend three feet above the upper landing surface, exposing employee to an elevated fall hazard of approximately 9 feet above a concrete surface.
