29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: a) East side of roof, south end to middle, on or about 10-29-21: Three employees were walking and working while performing roofing tasks on a flat roof and were not protected from falls, exposing the employees to a fall hazard of 50 feet. b) West side of roof, middle area, on or about 10-29-21: One employee was working while kneeling on top of a cornice/parapet section of the roof, and was not protected from falls, exposing the employee to a fall hazard of 52 feet.

29 CFR 1926.502(h)(1)(v): The safety monitor shall not have other responsibilities which could take the monitor's attention from the monitoring function: a) Entire roof of building, on or about 10-29-21: Three employees were walking and working while performing roofing tasks on a flat roof while being monitored by a Safety Monitor who was performing roofing work, with his back to the main work area, and out of view of all employees, without fall protection, exposing the employees to a fall hazard of 50 feet, and 52 feet.
