29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  a).  Building 5700: On or about 10/25/2021, the point of operation on the AFM Press Brake Model 5-55 was not guarded to prevent an employee from accessing the danger zone during cycling, as a result, exposed employee to a possible crushing and/or amputation hazard.  Note: Special hand tools for placing and removing material shall be such as to permit easy handling of material without the operator placing a hand in the danger zone. Such tools shall not be in lieu of other guarding required by this section, but can only be used to supplement protection provided.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i:    a)  Building 9A Weld Shop: On or about 10/27/2021, the compressed air nozzle, used by employees for cleaning purposes, was being used at 100 p.s.i. in lieu of using an approved nozzle or an inline regulator that would reduce the air pressure to less than 30 p.s.i  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.304(b)(3)(ii)(A): All 125-volt, single-phase, 15, 20 and 30-ampere receptacle outlets that are not part of the permanent wiring of the building or structure and that are in use by personnel did not have ground-fault circuit-interrupter protection for personnel:  a)   Building 9A, Duck work section: On or about 10/26/2021, an employee used multiple extension cords plugged together to supply power to a portable Porter Cable jigsaw to cut sheet metal without ground fault circuit interrupter protection, as a result, exposed employee to a possible shock hazard.   Note: A cord connector on an extension cord set is considered to be a receptacle outlet if the cord set is used for temporary electric power.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposed an employee to injury and the defective or damaged item was not removed from service and an employee was using it before repairs and tests necessary to render the equipment safe were made:   a)	Building 9A Weld shop: On or about 10/27/2021, the flexible cord on the Patton Air Circulator pedestal fan was not removed from service in that the male attachment plug was missing the ground prong, thereby exposed employees to a possible electrical shock hazard.  Note: If there is a defect or evidence of damage that might expose an employee to injury, the defective or damaged item shall be removed from service, and no employee may use it until repairs and tests necessary to render the equipment safe have been made.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
