29 CFR 1910.95(d)(3):The employer did not perform noise monitoring whenever a change in production, processes, equipment or controls may increase employee noise exposures:  a) Building 9A and 9B on 3487 Inventors Road, Norfolk, VA: On or around 10/27/21, recognized high noise processes, such as powder coating and welding, had not been monitored for personal noise dosimetry.  This is despite ongoing changes in the process, worksite, and personnel, which may have increased the noise exposure profiles.    b) Building 5700 Ward Ave, Virginia Beach, VA: On or around 10/29/21, recognized high noise processes, such as spray booth painting, abrasive blasting, and machining, had not been monitored for personal noise dosimetry.  This is despite ongoing changes in the process, worksite, and personnel, which may have increased the noise exposure profiles.    c) USS Washington at Newport News Shipbuilding:  On or about 11/01/21, recognized high noise processes, such as welding, grinding, and deck crawling, had not been monitored for personal noise dosimetry.  This is despite ongoing changes in the process, worksite, and personnel, which may have increased the noise exposure profiles.    Abatement required within 30 days after the abatement date.

29 CFR 1910.95(g)(6):The employer did not obtain a new audiogram annually for each employee exposed to noise at or above an 8 hour time weighted average exposure of 85 decibels:  a) Building 9A and 9B on 3487 Inventors Road, Norfolk, VA: On or around 10/27/21, employees performed routine work using processes and in areas measuring 85 decibels or higher in noise exposure. Annual audiograms were not maintained for these employees.  b) Building 5700 Ward Ave, Virginia Beach, VA: On or around 10/29/21, employees performed routine work using processes and in areas measuring 85 decibels or higher in noise exposure. Annual audiograms were not maintained for these employees.  c) USS Washington at Newport News Shipbuilding:  On or around 11/01/21, employees performed routine work using processes and in areas measuring 85 decibels or higher in noise exposure. Annual audiograms were not maintained for these employees.  Abatement required within 30 days after the abatement date.
