29 CFR  1926.405(g)(2)(iv):The employer shall ensure that flexible cords are connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws.  Near the bedroom window: The outer insulation on the extension cord used to charge tool batteries was pulled from the plug, exposing employees to fire and electrical hazards.
