29 CFR  1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  a) Production Area: On or about October 29, 2021, and at times prior, the employer exposed an employee to struck-by hazards, in that the employer did not develop and implement a control of hazardous energy program where an employee performs servicing and maintenance on the  "LS Industries" RBX-L Alloy Rim Blaster.  b) Production Area: On or about October 29, 2021, and at times prior, the employer exposed an employee to struck-by hazards, in that the employer did not develop and implement a control of hazardous energy program where an employee performs servicing and maintenance on the  "Lehigh Machinery" model MR-2540 lathe (serial number T0115010213).

29 CFR  1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy:   a) Strip Tank Area: On or about October 29, 2021, the employer exposed employees to electrical hazards, in that a wall-mounted metal outlet box containing a 110-volt electrical receptacle did not have a faceplate to protect employees who connect and disconnect the cord and plug of an in-wall fan to the receptacle to turn the fan on and off.

29 CFR  1910.36(h)(3):  Outdoor exit route(s) were not reasonably straight and did not have smooth, solid, substantially level walkways:  a) Exterior West Wall of Production Area: On or about October 29, 2021, and at times prior, the employer exposed employees to trip and fall hazards, in that the wooden landing at the bottom of the exterior stairs descending from the exit discharge was not maintained in a smooth, solid and level condition where the landing was not fully planked and the remaining wood was significantly deteriorated and broken.
